Armbruster, Esq., respectfully represents: 1. Jordan Raymond Ehrisman, an adult 22 years of age, was born to mentally, emotionally, and physically abusive parents and was thereby raised by his grandparents who provided a loving, nurturing, and stable home. 2. Jordan Ehrisman's grandparents in question are Harold Raymond Mcnaughton and Frances Ann Mcnaughton. 3. The name "Jordan Ehrisman" brings painful memories of childhood abuse to Jordan Ehrisman every time it is spoken or written. 4. Jordan Ehrisman, in addition to avoiding the mental anguish suffered by his current name, seeks to honor and carry on the name of said grandparents. 5. Jordan Ehrisman's wife, Kaitlin Grace Leblanc, seeks to join in this petition to also take the last name "McNaughton". • 6. Petitioners currently reside at 140 Bosler Avenue, Apt. B, Lemoyne, PA 17043. 7. During the five years preceding the filing of this petition, Petitioners have resided at: a. 1790 Hominy Drive, Newport, PA 17074; b. 2126 Turkey Bird Road, Newport, PA 17074; c. 2500 Tahoe Lane, Denton, TX 76210; d. 25 South 8th Street, Lemoyne, PA 17043. 8. That there are no judgments or decrees of like character of record against the Petitioners. WHEREFORE, Petitioners requests the Court, pursuant to 54 Pa.C.S.A. 701 et seq., for the following: a. To fix a hearing on this Petition; b. To direct that notice be given of this Petition and hearing; and c.
