TO THE HONORABLE, THE JUDGES OF SAID COURT
The Petitioner AMBER LEIGH EBERHART, by and through her attorneys, Andrews and
Johnson and Taylor P. Andrews, Esquire, request that this Court Order a change of name for her
pursuant to 54 Pa. CONS. STAT. Section 701 et. seq. and in support thereof respectfully represents as
follows:

Petitioner is an adult individual having been born October 12, 1982.
Petitioner AMBER LEIGH EBERHART, is a resident of Cumberland County,
Pennsylvania and has resided at the following locations during the five years next proceeding the filing
of this petition:
33 Victoria Way, Camp Hill, PA 17011 from September 1997 to present; and
503 Erfbrd Rd., Camp Hill, PA 17011 from February 1996 until September 1997; and
722 North Market Street, Selinsgrove, PA 17870 from 1994 to February 1996.
Petitioner requests that her name be changed from Amber Leigh Eberhart to Amber Leigh
DeBarr.
4. The reason for the requested change of name is that Petitioner desires to assume the
maiden name of her mother. Petitioner's father has not been involved in Petitioner's life.
WHEREFORE, Petitioner respectfully requests that this Court enter an Order changing her name from
Amber Leigh Eberhart to Amber Leigh DeBarr.
