Martha J. Beers (the "Decedent"), a widow, died on October 31, 2009, at the age of 72. An original Death Certificate is attached hereto as Exhibit "A" and made a part hereof. 2. Decedent died with a Last Will and Testament dated November 26, 1986 (the "Will"). The original Will is attached hereto as Exhibit "B" and made a part hereof. 3. The witnesses to the execution of the Will, Ronald E. Beers and Andrea A. Beers, have executed an Oath of Subscribing Witness. The original Oath is attached hereto as Exhibit "C" and made a part hereof. 4. At the time of her death, Decedent's last principal residence was at 29 Adams Street, East Pennsboro Township, Enola, Cumberland County, PA 17025. 5. Decedent is survived by the following descendants: Debra Beers (daughter) Mary Beers (daughter) 29 Adams Street 29 Adams Street Enola, PA 17025 Enola, PA 17025 Val Parr (daughter) Virginia Cox (daughter) 233A Market Street (In the process of attempting to locate.) Mechanicsburg, PA 17055 6. Decedent owned real estate located at 29 Adams Street, East Pennsboro Township, Cumberland County, Pennsylvania and had an account with M&T Bank at the time her death. 7. Under the Will, Petitioner is named successor Executrix of the Estate due to Carl K. Beers' death on October 5, 2009. 8. Under the Will, the named beneficiaries have the following interest: Name Debra Beers Mary Beers Val (Parr) Virginia (Cox) WHEREFORE, Petitioner respectfully requests that the Register of Wills issue a Citation upon Mary Beers, Val Parr, and Virginia Cox to show cause why Letters Testamentary should not be granted to Debra Beers under 20 Pa.
