avers the following:
Stephanie E. Chertok, Intervenor, is an attorney and a registered nurse licensed in
the Commonwealth of Pennsylvania.

2. Intervenor is interested in the welfare of Mimi Blazer and she has been working to
seek restitution for fraud that has been perpetrated upon Ms. Blazer over many years by the
individual who operated under a Power of Attorney that was granted by Ms. Blazer in 1998.
3. Intervenor has sought to have criminal charges brought against the Power of
Attorney based on the knowledge and belief that the Power of Attorney has been receiving
payments from a trust fund on behalf of the incompetent but he has failed to make those funds
available for the care of the incompetent. Based on knowledge and belief, Intervenor has made
repeated attempts to have the District Attorney investigate and prosecute the Power of Attorney
for converting the incompetent's funds for his own use.
4. Intervenor has engaged in discussions with the Cumberland County Office of
Aging in an effort to engage their services on behalf of the incompetent, asserting that the Office

AND NOW COMES the Petitioner, Stephanie E. Chertok, Esquire, ("Int~'Venor"), ~l
of Aging should assist in causing the District Attorney to take action, and should seek to have
Intervenor appointed as Guardian for the incompetent.
5. While waiting for the response of the Office of Aging, Intervenor learned of the
pending guardianship hearing on Sunday, December 27, 2009 and as a result Intervenor files this
petition.
6. Intervenor concurs in the request to have a guardian appointed for the
incompetent individual; however, Intervenor opposes the appointment of the proposed guardian
company located in Elizabethville, Pennsylvania.
7. Intervenor seeks to be the person to be appointed by the court as guardian of the
incompetent due to the fact that Intervenor is intimately familiar with the history and the needs of
the incompetent individual.
g. Intervenor has been working for the alleged incompetent for several years in order
to see to it that her needs are met and Intervenor will continue to do so as she will pursue
criminal charges and restitution against the Power of Attorney that allegedly stole monthly
payments from the incompetent's trust fund for many years and kept them for his own use.
9. Intervenor is in the best position to act as guardian for the alleged incompetent
individual as she lives and works within one mile of the facility where the alleged incompetent
lives, she is an attorney that can best protect and defend the incompetent individual in any legal
proceedings that shall be brought concerning the alleged incompetent, and she is a registered
nurse in the best position to discuss the medical needs of the alleged incompetent individual with
health care providers and to act as the Power of Attorney for medical decisions pursuant to the
Guardianship.
10. The alleged incompetent individual has had no guardian appointed previously.
11. The proposed guardian is a corporate entity located Elizabethville, Pennsylvania
and has no personal knowledge of the history and the personal needs, medical needs and legal
needs of the alleged incompetent individual.
12. Intervenor seeks to be the appointed guardian of the person and the Estate of
Mimi Blazer because she is the only person who shows both a genuine ongoing interest in the
alleged Incompetent's welfare and has demonstrated a willingness to assume the role of
guardian.
WHEREFORE the Intervenor, Stephanie E. Chertok, prays the court to appoint her as
primary guardian of both the Estate and the person of Mimi Blazer, adjudged incompetent
individual.
