I am already known by my orooosed name in my profession. and I would like to use this name in all my
personal and business dealings.
6. The Petitioner has not been convicted of a felony.
WHEREFORE, the petitioner respectfully requests that this Court enter an Order
changing his/her name from Hee Ri Kang to Hayley Heed Kang
