GONZALEZ NO. - 330 ',vi Terw PETITION FOR CHANGE OF NAME TO THE HONORABLE JUDGES OF THE COURT: The Petition of Nathaniel R. Gonzalez a/k/a Nathanal R. Gonzalez, filed by and through his counsel, Linda A. Clotfelter, Esquire, respectfully sets forth the following: 1. Petitioner is Nathaniel Richard Gonzalez a/k/a Nathanal Richard Gonzalez, an adult individual who resides at 6 Kim Acres Drive, Mechanicsburg, Cumberland County, Pennsylvania 17055. 2. Petitioner has always been known as Nathaniel R. Gonzalez despite the fact that his birth certificate indicates that his name is Nathanal R. Gonzalez. 3. Petitioner's natural father is Richard Everett Curtis and his natural parents were never married to each other. 4. Petitioner wishes to have his name reflect his true identity and heritage. He is engaged to be married and wishes to have any children or other descendants bear his family name. Thus, Petitioner seeks to have his name changed to Nathaniel Richard Curtis. 5. There are no judgments or decrees of like character entered against Petitioner. WHEREFORE, Petitioner, Nathaniel R. Gonzalez a/k/a Nathanal R. Gonzalez, respectfully requests that this Court, pursuant to the Act of December 16, 1982, P.L. 1309, No. 295, § 2, 54 Pa. Cons. Stat. Ann. § 702, do the following: a. Schedule a hearing on this Petition; b. Direct that notice be given on this Petition and hearing; and c.
