On Se tember 25, 2009, Judge Edward Guido appointed the Cumberland~ounty Cice
p
of Aging as temporary emergency Guardian of Person for Pauline Demler. Following the
appointment, Pauline was admitted to Claremont Nursing and Rehabililtation. On
October 16, 2009 the Cumberland County Aging and Community Services was
appointed permanent Guardian of Person. Her medical conditions at time of admission
were coronary artery disease, peripherial vascular disease, ,diabetes milletus,
hypercholesteral, hypertension, ischemic cardiomyopathy, gangrene right third toe,
history of myocardia Infarction (heart attack). She was later diagnoses with demenm a.
with delirium, depression, allergic rhinitis, paranoia, hallucinations and chronic ane

Medications she is receiving are insulin, amlodipine for blood pressure, aspirin for h cart,
isosobide for the cardiomyopathy, lisinopril for blood pressure, loratadine ofr allerg e ,
plavix for CAS, risperiodine for behavioral issues, sertaline for depression, ariceptnoas
dementia, simvastatin for cholesterol, albuterol for wheezing and ibuprofen for pai
needed.
The day following the temporary hearing, Ms. Demler had surgery to remove a toe due
to infection. Later she also had two teeth extracted. Ms. Demler seems to have adjusteh
well to Claremont .She likes the food, the staff and the activities that she has alongs ns
other residents. She appears to be very social and is out of her room a lot. Her two
that live in the area had been visiting her rather frequently when she was first admitted
but they later stopped. The one son had not seen his mother in 8 years until the time
around the hearing. She receives letters from another son who is in prison.
It is the request of the Cumberland County Office of Aging that the agency retain
Guardianship of Person for Pauline Demler. The Claremont Nursing and Rehabilitation
has demonstrated an adequate quality of care for her medical and mental problems. We
believe, therefore, that Ms. Demler should continue to reside in that nursing facility.
