AND NOW, this ~~ day of Jaw , 2010, upon consideration of the
Petition for Appointment of a Permanent Plenary Guardian of the Person and Estate of
Dennison H. Baker pursuant to 20 P.S. Section 5513, and following a hearing at which
Dennison H. Baker was present, and at which he was represented by Court appointed
counsel, Mark F. Bayley, Esquire, and at which evidence was presented by the Petitioner
in support of the Petition, and, based upon the foregoing, the Court finds:
1. That Dennison H. Baker is an incapacitated person in that he puffers from
a mental impairment that prevents him from receiving and evaluating information
effectively and prevents him from formulating and communicating decisions to such a
significant extent that he is totally unable to manage his financial affairs or meet essential
requirements for his physical health and safety on a daily basis, and is adjudicated as
such.
2. That the Court appoints his brother, Jeffrey W. Baker, as Permanent
Plenary Guardian of the Person and Estate of Dennison H. Baker. No bond shall be
required to be filed. ~

3. That the Guardian shall file on a timely basis with the Court all reports and
accounts required by statute.
