Thrush Dr. Carlisle, PA 17015 The following, abng with the enclosures, is the final report regarding the estate and person of Charles C. Awkerman, per our appointment as guardians. Please be advised that Charles died on October 30, 2009 Until his death, Charles continued to reside in the Alzheimer's Wing of the Satan Todd Memorial Home at 1000 W. South St., Carlisle, PA, 17013. Since our last report in September 2009, his mental capacity had not changed and he remained fully dependent on the nursing home staff for his care and well being. His overall state of heahh had otherwise not changed until his final illness, which began two days prior to his death Since our last report and until the time of his death, we continued to visit him frequently and provide for his needs in the following ways: • Washing and ironing all of his personal laundry. Mending as needed. Providing his clothing and other personal items as needed. • Cutting his fingernails regularly. • Attending dental and specialist appointments to assist staffas he would not cooperate and allow care to be provided unless one of us was there with him. • Taking his 20-month-old great-granddaughter to visit him several times, when she is here from Blue Bell. • Attending quarterly care plan conferences. • Taking numerous phone calls from nursing home staff who were reporting various issues, concerns, and problems. Each of these calls involves discussing Charles's care and treatment with nursing home staff. • Speaking with Dr. Dell (his primary care provider) periodically to find out his assessment of Charles's condition, to request needed physician orders, and to provide information regarding Charles's condition that we have become aware of. • Handling all ofhis financial matters, including paying bills. • Spending several days at his bedside during his last illness providing comfort and care as needed. We feel we fulfilled Charles's needs lovingly and willingly, and we have executed the terms of the guardianship with the full support of our other two sisters, Pamela B. Burns anti Emily J, Awkerman.
