FINAL GUARDIANSHIP REORT i ~ ; ; ~.. } ~yT'erry I~rIeY :: ; ~ --, ,; Director .. .- FOR NANCY HOERNER ~ ~``=' -'' ~ '_~ January 22, 2010 ~`~' ~~ ~~` Report from Guardian of Estate cam, Cumberland County Office of Aging Janet Paull, Aging Care Manager 3 On April 6, 2006 Judge Kevin Hess appointed the Cumberland County Office of Aging the Permanent Plenary Guardian of Estate for Nancy Hoerner. Ms. Hoerner had a checking account and a Christmas Club account at Sovereign Bank. Following the hearing the Office of Aging opened a guardianship account for her at Sovereign Bank. Subsequently, the Office of Aging became Ms. Hoerner's Representative Payee for her Social Security. Her social security is her only income and her check is directly deposited into her guardian account at Sovereign Bank. Currently Ms Hoerner's monthly income is $871.00. Ms. Hoerner is a resident at South Mountain Restoration Center which is a state run nursing home. The monthly payments of $743.78 are made out to the Commonwealth of Pennsylvania. In June 2008 the Office of Aging petitioned for and was granted permission to receive a monthly guardianship fee of 10% his gross income. Currently the amount is $82.30 In addition, there were some payments made for personal items, supplies, clothing, cigarettes and spending money. Since Nancy is on Medical Assistance, she can only retain $45.00 of her income with the remainder going to the Home. Each month $40 is sent to the home for her personal trust account. On December 22, 200 9 Judge Hess signed an order transferring the guardianship of Ms. Hoerner to Franklin County Area Agency on Aging since that is the county where she resides. At the time of the transfer the balance in her checking account at the time of the transfer was $445.68. Attached is a financial summary of Nancy Hoerner's income and expenses. Also attached is a spread sheet representing her checking account register.
