PETITION FOR CHANGE OF NAME
AND NOW, this 0 day of February, 2010, comes Petitioner, Curtis Leon Charles J&kins
and files the following Petition, and in support thereof, avers the following:
1. Petitioner, Curtis Leon Charles Jenkins, is an adult individual residing at 1301
Woodward Drive, Carlisle, Pennsylvania, Cumberland County, Pennsylvania.
2. Petitioner was born July 15, 1949, in Manhattan, New York.
3. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six (6)
months previous to the filing of this Petition.
4. Petitioner desires to change his name from Curtis Leon Charles Jenkins to Abdul
Rahmaan Arraheem.
5. Petitioner is seeking a change of name for the following reasons:
a. Petitioner has used the name Abdul Rahmaan Arraheem since he converted to
Islam in the 1970s.
b. Although Petitioner converted back to Christianity, he continues to use and be
known by the name Abdul Rahmaan Arraheem.
C. Petitioner's wife and children use the surname Arraheem-Waters, which is a
hyphenation of Petitioner's wife's maiden name, Waters, and Petitioner's desired surname.

6. Petitioner does not wish to change his name for any illegal purpose, to avoid the
payment of taxes or other debts, or to avoid any obligation. Petitioner has been fingerprinted and his
fingerprint card is attached hereto as Exhibit "A" and by reference incorporated herein.
7. Petitioner's counsel has searched the index in the Prothonotary's Office in the Court
of Common Pleas of Cumberland County and there are no judgments against Curtis Leon Charles
Jenkins.
WHEREFORE, Petitioner, Curtis Leon Charles Jenkins, respectfully requests this Honorable
Court to issue an Order changing his name from Curtis Leon Charles Jenkins to Abdul Rahmaan
Arraheem.

2. Movant's name was changed from Curtis Leon Charles Jenkins to Abdul Rahmaan
Arraheem, by Order of Court dated March 31, 2010.
3. Movant has applied for an updated copy of his birth certificate from the State of New
York.
4. The State of New York has rejected his request for a birth certificate because they
require that the order changing Movant's name include the date of birth and place of birth of the
Movant.

MOTION FOR ENTRY OF AMENDED ORDER
AND NOW, this 25t h ay of January, 2011, comes Movant, Abdul Rahmaan Arraheem,
formerly known as Curtis Leon Charles Jenkins, and files the following Motion, and in support
thereof, avers the following:
1. Movant, Abdul Rahmaan Arraheem, is an adult individual residing at 1301
Woodward Drive, Carlisle, Pennsylvania, Cumberland County, Pennsylvania.
2. Movant's name was changed from Curtis Leon Charles Jenkins to Abdul Rahmaan
Arraheem, by Order of Court dated March 31, 2010.
3. Movant then applied for an updated copy of his birth certificate from the State of New
York.
4. The State of New York rejected his request for a birth certificate because they
required that the order changing Movant's name include the date of birth and place of birth of the
Movant.
5. On September 1, 2010, Movant filed a Motion for Entry of Amended Order.
6. An Amended Order of Court was entered on September 3, 2010, adding the date and
place of birth of Movant.
7. On September 10, 2010, Movant again applied for an updated copy of his birth
certificate from the State of New York.
8. The State of New York again rejected Movant's request for a birth certificate because
the State of New York revised its requirements sometime between September and December, and it
is now required that the order changing Movant's name include his parents' names and his birth
certificate number. A copy of a letter dated December 29, 2010, from the Department of Health,
Division of Vital Records is attached hereto as Exhibit "A".
9. The Honorable Edward Guido has been assigned to this matter.
WHEREFORE, Movant, Abdul Rahmaan Arraheem, respectfully requests this Honorable
Court issue the attached Amended Order of Court to include his parents' names and his birth
certificate number.
