North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners of the property against which
this claim is filed is Rodney Young, 783 Longs Gap Road, Carlisle, PA 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 783 Longs Gap Road, Carlisle, PA 17013.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer user fees against the above-described property in the amount of
$258.60, plus any additional fees incurred thereafter.

Interest is claimed on the above-mentioned fees until paid in full.

BEGINNING at a point in the center line of Long's Gap Road, Pennsylvania, Legislative Route No.
21072, said point being 120 feet South of the southem.line of Hilicrest Avenue; thence extending along
land now or formerly of Wayne Caleman, Being Lot No. 12 on the hereinafter mentioned plan of lots
North 72 degrees 30 minutes East 160 feet to a point in line of land of Robert Carpenter, thence along
said land South 17 degrees 30 minutes East 60 feet to a corner of land of James Eberly; thence along said
land being Lot No. 14 on said Plan, South 72 degrees 30 minutes West 160 feet to a point in the center
line of Long's Gap Road aforesaid; thence along said center line North 17 degrees 30 minutes West 60
feet to the point and place of BEGINNING.
BEING Lot No. 13 on a plan of lots laid out by Amos D. Keck which said plan is recorded in Plan Book
4, Page 52, Cumberland County Records.
HAVING thereon erected a one-story frame dwelling.
BEING known as 783 Longs Gap Road, Carlisle, PA.
Tax ID No. 29-15-1247-009.
BEING the same premises which Washington Mutual Bank, by its attorney-in-fact, Option One
Mortgage, by Deed recorded 2124/2003, in the Office of the Recorder of Deeds in and for the County of
Cumberland, Pennsylvania, in Book 255, Page 4294, granted and conveyed unto Richard L. Burkholder,
grantor herein.
