MUNICIPAL LIEN FOR SEWER SERVICES
North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners of the property against which
this claim is filed is Mary Zayas-Bazan, 154 Faith Circle, Carlisle, PA 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 154 Faith Circle, Carlisle, PA 17013.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.
The lien is claimed for unpaid sewer user fees against the above-described property in the amount of
$385.77, plus any additional fees incurred thereafter.

Interest is claimed on the above-mentioned fees until paid in full.

Cumberland County, Pennsylvania, hereinafter referred to as
GRANTEE
WITNESSETH, that in consideration Sixty Eight Thousand Four Hundred
and Fifty ($68,450.00) Dollars,in hand paid, the receipt whereof is hereby
acknowledged, the said Grantors do hereby grant and convey to the said
Grantee, her heirs and assigns:
ALL that certain tract of land, with the improvements thereon erected, situate
in North Middleton Township,Cumberland County, Pennsylvania, bounded
and described as follows:
BEING Lot No. 29 on the Plan of Kingsbrook, Section "T', as recorded in the
office of the Recorder of Deeds for Cumberland County, in Plan Book 27, Page
3; containing 50 feet along the east along Faith Circle; containing 200 feet
along the north along Lot No. 30 on said Plan; containing 50 feet along the
west along land now or formerly of Hooke, Lebo & Hooke and containing 2011
feet along the south along Lot No. 28 on said Plan and containing 10,000.00
square feet.
BEING improved with a brick and aluminum three bedroom duplex known
as 154 Faith Circle, Carlisle, Pennsylvania 17013.
BEING the same premises which Archer Kent Blood and Margaret Millward
Blood, his wife, granted and conveyed unto Jay F. Vogel and Kathryn A,

Vogel, his wife, by deed dated June 7, 1985, and recorded in the Cumberland
County Recorder of Deeds Office in Deed Book 31 H 1031.
BEING SUBJECT to the building; and use restrictions as recorded in the Office
aforesaid in Miscellaneous Book No. 184, page 763.
AND the said Grantors will warrant Generally the property hereby conveyed.
