Petitioner, Eric Richard Dellape, is an adult individual residing at 152 C Street, Carlisle, Cumberland County, Pennsylvania 17013. 2. Petitioner was born on October 14, 1981 in Blair County, Pennsylvania. 3. At the time of birth, Petitioner was given the name Eric R. Epple. He was thereafter adopted by his step-father and given the surname "Dellape." A copy of the birth certificate evidencing this fact is attached hereto and incorporated herein as "Exhibit A." 4. The petitioner is not married. 5. Petitioner is employed by the Pennsylvania State Police as a Troooper assigned to the Carlisle Barracks. 6. There are no judgments or decrees of record against Petitioner. 8. Petitioner now desires to have his name changed to Eric Richard Griswold, since he has no family connection to his present surname and the Griswold surname is his mother's maiden name. WHEREFORE, Petitioner requests your Honorable Court to enter an Order granting the Petitioner's request to change his name from Eric Richard Dellape to Eric Richard Griswold.
