Haggerty, Esquire, of Aborn & Kutulakis, LLP, on behalf of the Petitioner, Ryan Martin Wilson, who respectfully requests this Court allow him to change his surname to Ryan Martin Wilson Tilden, and in support thereof avers as follows: 1. Petitioner is Ryan Martin Wilson, an adult individual who resides at 22 Buttonwood Lane, Carlisle, Cumberland County, Pennsylvania. 2. A Petition for Adoption filed by David Tilden requesting to adopt Petitioner is currently before This Honorable Court for disposition. 3. A Hearing on the within Petition will be held at the same time as the final adoption proceedings. 4. Petitioner desires to change surname by adding his adoptive father's surname, Ryan Martin Wilson Tilden. 5. There are no judgments or decrees of record against the Petitioner. 6. Attached to this Petition is a fingerprint card containing the fingerprints of the Petitioner. The Petitioner's fingerprints were placed on the fingerprint card on May 25, 2004. Said fingerprints are attached for the purpose of forwarding the same to the Pennsylvania State Police as required by 54 Pa.C.S. ~ 702(b). WHEREFORE, petitioner respectfully requests this Honorable Court to enter an Order granting the petitioner's request to change his name from Ryan Martin Wilson to Ryan Martin Wilson Tilden.
