C.S. 701 and 02 AND NOW comes the Petitioner, ERIC FRANCIS WOODWARD, joined by his wife, CASSANDRA RAE STEELE, by and through their undersigned attorneys, and jointly petition for a change of surname, stating in support the following: 1. The petitioners are Eric Francis Woodward and Cassandra Rae Steele, husband and wife. 2. The petitioners intend to change their surnames to MIZAK, the surname of Mr. Woodward's maternal grandparents. 3. Mr. Woodward currently bears the surname of his biological father, Wayne F. Woodward. 4. Mr. Woodward, now age twenty-eight years, has been emotionally estranged from his biological father since the age of ten years, and has had no contact at all with his biological father for at least the past six years. 5. Mr. Woodward enjoys close family ties with his maternal grandparents and desires to adopt their surname, "Mizak." 6. Ms. Steele, who is married to Mr. Woodward, is pregnant with the petitioners' first child, expecting to give birth at the end of March of this year, and she wishes to take the surname Mizak for herself and her child. 7. The petitioners currently reside at 1000 Belle Vista Drive, East Pennsboro Township, with Enola mailing address, Cumberland County, Pennsylvania. 17025. 8. Prior to purchasing their current residence in August of 2009, the petitioners both resided at 507D Ross Avenue, New Cumberland, PA 17070 for the preceding two years and seven months. 9. Prior to residing at 507D Ross Avenue, New Cumberland, PA, Mr. Woodward resided at 1424 Yorktown Road, Mechanicsburg, PA 17050. 10.
