AFFIDAVIT OF SERVICE
I, Todd F. Truntz, Esquire, an attorney with the law firm of Keefer Wood Allen &
Rahal, LLP, being duly sworn according to law, depose and state that I served a copy of
the Citation and Petition for Appointment of a Plenary Guardian of the Estate of an
Alleged Incapacitated Person Pursuant to 20 Pa. C.S.A. § 5511 et. seq. and to Modify
Revocable Trust Pursuant to 20 Pa. C.S.A. § 5536, in the above matter upon Harry W.
Preis, an Alleged Incapacitated Person by personal service on Tuesday, March 9, 2010,
and that the Petition and Citation were explained to the maximum extent possible in
language likely to be understood by the alleged incapacitated person and that a copy of
each was left with him.
