Attached hereto is a statement of the kind and character of the services so rendered and

the prices charged therefor, said statement being a true and correct copy of the Owner's account
with the Claimant, which sum of $1,179.98 was duly assessed against the above Owner and
premises beginning April 1, 2008 and for which sum, which includes interest, costs and
penalties, a lien is claimed against the above premises in accordance with the Municipality
