Plaintiff State Farm Mutual Automobile Insurance Company is an insurance carrier licensed to conduct business in the Commonwealth of Pennsylvania and having as one of its principal places of business the above-captioned address. 2. Earl Goshea is an adult and on or about May 16, 2008, was the registered owner of a 2005 Ford Ranger motor vehicle insured under a policy of .-6 insurance issued by State Farm Mutual Automobile Insurance Company, which included Uninsured Motorist Bodily Injury Coverage (`UMBI"). Defendant Ann Marie Oharen is an adult residing at 1232 Scenery Drive in Mechanicsburg, PA 17050 and on or about May 16, 2008 was the registered owner of a 2005 Lexus RX 330 motor vehicle. 4. Defendant David J. Oharen is an adult residing at 1232 Scenery Drive in Mechanicsburg, PA 17050 and on or about May 16, 2008 was the authorized operator of the aforementioned Lexus RX 330 motor vehicle. 5. On or about May 16, 2008, Defendant David Oharen was operating the aforementioned Lexus motor vehicle on SR 581 in Lemoyne Borough, Pennsylvania, which struck Plaintiff's insured's vehicle in the rear. 6. The aforesaid collision also caused Plaintiff's insured, Earl Goshea, to suffer severe, painful, serious and permanent bodily injuries to their person. 7. At the time of the aforesaid accident, the vehicle owned and operated by the Defendant(s) was uninsured resulting in the presentment of UMBI claims to Plaintiff by Earl Goshea. 8. Plaintiff's insured was paid $10,000.00 in settlement of his "UMBI" claim. 9. The aforesaid accident was a direct and proximate result of Defendant(s) negligence, carelessness, recklessness and/or other liability-producing conduct. 10. Pursuant to the insurance policy herein mentioned, Plaintiff State Farm Mutual Automobile Insurance Company a/s/o Earl Goshea has incurred and paid expense and other damages in the amount of $10,000.00 WHEREFORE, Plaintiff State Farm Mutual Automobile Insurance Company a/s/o Earl Goshea demands judgment against the Defendant in the amount of $10,000.00, which includes the Plaintiff s insured's deductible, plus interest, attorneys' fees, and costs of suit.
