Owner and described. as follows:

for unpaid sewer and/or trash services rendered from April 1, 2008 to January 31, 2010. Said
services were rendered and charged pursuant to Hampden Township Code of Ordinances
Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended.
Attached hereto is a statement of the kind and character of the services so rendered and
the prices charged therefor, said statement being a true and correct copy of the Owner's account
with the Claimant, which sum. of $1,145.29 was duly assessed against the above Owner and
premises beginning April 1, 2008 and for which sum, which includes interest, costs and
