Thomas ("Owner") and all that certain tract of land including improvements thereon, owned by the Owner and described as follows: 104 North 36`h Street Camp Hill, PA 17011 Tax Property Map: 10-21-0275 Parcel No.: 063 for unpaid sewer and/or trash services rendered from October 1, 2008 to January 31, 2010. Said services were rendered and charged pursuant to Hampden Township Code of Ordinances Chapter 18, Part 2 and/or Chapter 20, Part 1, as amended. Attached hereto is a statement of the kind and character of the services so rendered and LAW OFFICES SNELBAKER & BRENNEMAN, P.C. the prices charged therefor, said statement being a true and correct copy of the Owner's account with the Claimant, which sum of $885.31 was duly assessed against the above Owner and premises beginning October 1, 2008 and for which sum, which includes interest, costs and penalties, a lien is claimed against the above premises in accordance with the Municipality F /Q, d Pd &j?y c? ?S3 7 Authorities Act of June 19, 2001, P.L. 287, 53 P.
