HUNTER : CIVIL ACTION - LAW :NO: 2010- Illy C,,ff'? PETITION FOR CHANGE OF NAME Petitioner, Donald J. Hunter, by and through his attorney, Nathan C. Wolf, Esquire, hereby presents this petition requesting that this Court order a change of petitioner's name, pursuant to 54 Pa. C.S. 5 701 et seq., and in support thereof makes the following averments: Petitioner was born on November 20, 1991, and is over the age of 18 years. 2. During the five years preceding the filing of this petition down to and including the date thereof, petitioner has resided continuously at the following locations: (a) 734 Trail Lane, Enola, Pennsylvania 17025 - August 2006 -Present; (b) 97 Meade Drive Gettysburg, Pennsylvania 17325 - 2004-2006; and, 3. There are no judgments or decrees of record or any other matters of like character against the petitioner. Certification of an official search is attached hereto as Exhibit «A„ 4. Petitioner requests that this name be changed from Donald J. Hunter to Donald Frederick Brandewie. 5. The reasons for the requested change of name are the following: (a) Petitioner surname and middle name were given in honor of Petitioner's father, who has had little to no relationship with the Petitioner for the past eleven years. spa, od P°? ?y ,ctt a-3y s'37 ,.1&p rr.,I '"-' -'6 XP (b) By contrast, Petitioner has had an extremely close relationship with his grandfather, Donald Frederick Brandewie, and wishes to change his middle and surname to match his grandfather's name to ensure the continuation of the family name and out of a sense of honor and respect for his grandfather. (c) Petitioner avers that his grandfather has always fulfilled the father figure role in his life; (d) The timing of Petitioner's request is being made now that he has reached the age of majority and he wishes to change his last name prior to reaching many milestones which will require documentation of his name, and sought to have this change occur without the necessity of changing these documents in the future. If approved, Petitioner's request will merely change the middle and family name but not Petitioner's first name, which was already given in honor of his grandfather. WHEREFORE, petitioner, Donald J. Hunter, respectfully prays that this Honorable Court enter an order fixing a time for a hearing on the instant petition before this Court, no less than one month, nor more than three months from the filing of the instant petition, and that after hearing, that this Honorable Court issue an Order authorizing the change of petitioner's name to Donald Frederick Brandewie, along with any additional relief that the Court may deem appropriate.
