CIVIL ACTION - COMPLAINT Plaintiff is a corporation licensed and authorized to conduct business in the State of Kansas with a place of business located at 1674 Industrial Highway, Ottawa, Kansas 66067. 2. Defendant is a business organization licensed and authorized to conduct business in the Commonwealth of Pennsylvania and having as one of its places of business 429 South 18"' Street, Camp Hill, Pennsylvania 17011. On or about February 4, 2008 Defendant entered into a Contract with Plaintiff at the specific instance and request of Defendant in which Plaintiff provided certain items more fully identified in the Contract to Defendant. See attached as Exhibit 1, a copy of the Contract. 4. Defendant breached the Contract by failing to make the agreed upon payment to Plaintiff, despite Plaintiff's compliance with the terms of the Contract. Despite demand for payment, Defendant refuses to pay the amount owed and has an outstanding balance in the amount of $10,100.00.
