AND NOW, comes Kara W. Haggerty, Esquire, on behalf of the
Petitioner, Tiffany Louise Myers, who respectfully requests this Court allow her
to change her name to Tiffany Louise Carver, and in support thereof avers as
follows:
1. Petitioner is T.L. Myers, a child individual with a date-of-birth of
March 5, 1999, who resides at 7 North High Street, Newburg, Cumberland
County, Pennsylvania.
2. At the time of her birth, Petitioner was given the name Tiffany
Louise Myers.

3. Petitioner desires to change her name to Tiffany Louise Carver.
4. Petitioner desires to change her name to Tiffany Louise Carver to
match her birth Mother's name, and maternal grandfather's name.
5. Over the past five years petitioner has resided in the following
locations:
a. 7 North High Street, Newburg, PA 17240
b. 148 "A" Street, Carlisle, PA 17013
6. There are no judgments or decrees of record against petitioner.
7. Attached to this Petition is a fingerprint card containing the
fingerprints of the Petitioner. The Petitioner's fingerprints were placed on the
fingerprint card on April 2, 2010 at the Pennsylvania State Police, Carlisle,
Pennsylvania. Said fingerprints are attached for the purpose of forwarding the
same to the Pennsylvania State Police as required by 54 Pa.C.S. §702(b).
8. Petitioner has had no contact with her father, Lee Allen Myers, in
more than three (3) years and has no method of contacting him. It is believed
that Mr. Myers had last resided in Mt. Holly Springs, Pennsylvania, but
Petitioner does not have a last known address.
WHEREFORE. Petitioner respectfully requests this Honorable Court
to enter an Order granting the Petitioner's request to change her name from
Tiffany Louise Myers to Tiffany Louise Carver

Louise Myers to change her name to Tiffany Louise Carver, and in support thereof
avers as follows:
1. Petitioner is Ronda Carver, natural mother of T.L. Myers, a child
individual with adate-of--birth of March 5, 1999, who resides at 7 North High Street,
Newburg, Cumberland County, Pennsylvania.
2. At the time of her birth, T.L. Myers was given the name Tiffany Louise
Myers.
3. Petitioner desires to change her daughter's name to Tiffany Louise
Carver.
4. Petitioner desires to change her daughter's name to Tiffany Louise
Carver as it is her daughter's desire to have her name match her birth Mother's name
and her maternal grandfather's name.
5. Over the past five years petitioner has resided in the following locations
with her daughter:
a. 7 North High Street, Newburg, PA 17240
