VERIFICATION OF SERVICE
I, Patricia A. M. Havens, hereby verify and state that:
I am the undersigned Petitioner for Guazdianship of Susan J. Myers, an alleged
incapacitated person, and am therefore authorized the make this verification.
2. On March 19, 2010 I personally served a copy of the Petition for Guazdianship,
accompanying Citation with Notice, and Judge Oler's March 7, 2010 Preliminary Decree on the
alleged incapacitated person, Susan J. Myers. I reviewed the Petition and Citation with Ms. Myers,
and then, to the maximum extent possible, explained the documents to her, in language and terms
she was most likely to understand.
3. The facts contained in this verification are true and correct to the best of my
knowledge, information and belief.
4. I understand that false statements herein are made subject to the penalties of 18 Pa.
C.S. § 4904, relating to unsworn falsification to authoriie
