PETITION FOR NAME CHANGE
AND NOW COMES, Susan Marie Rubino, by and through his attorney, Lori K.
Serratelli, Esquire, and the law firm of Serratelli, Schiffman & Brown, P.C., who files this
Petition pursuant to 54 Pa. C.S.A. §701(a.1) and avers as follows:
1. Petitioner, Susan Marie Rubino, born August 30, 1954 in Allendale, South
Carolina, currently resides at 107 Pepper Avenue, Enola, PA 17025.
2. For the last five years, Petitioner has resided at:
107 Pepper Avenue, Enola 2008 to Present
3331 Whetstone Road, North, South Carolina 2005 to 2008
3. On February 1, 2005, Dr. Michael Brownstein of 1001 Marisposa Street, Suite
101, San Francisco, California performed an irreversible surgical procedure for the purpose of
altering Petitioner's sex characteristics from female to male. .19.7.6 6 ?d- 194k

4. Petitioner has completed her gender reassignment surgery and has been known by
the name of Chris Rubino since 1972.
5. Petitioner has used the name of Chris Rubino consistently, non-fraudulently and
exclusively since 1972.
6. Petitioner desires to have her birth certificate changed to reflect that she is now a
male and that her name is Chris Carrara Rubino.
7. Petitioner is judgment free and is not seeking to change her name to avoid any
financial obligations or commit fraud and there is no public interest is harmed by the approval of
the name change. Maverick McIntyre, 715 A2d 400, 552 Pa. 324 sup. 1998.
8. The power of the Court to order a name change in the instance circumstances and
to order a change of birth records was recognized in the case of In Re: Dickinson, 4 Pa. DNC.
P 678 (1978).
9. Attached hereto are Petitioner's fingerprints.
WHEREFORE, Petitioner prays this Honorable Court order that Susan Marie Rubino
shall hereafter be known as Chris Carrara Rubino and that her birth certificate shall be changed
to reflect she legally is a male.
