Whitman, Aging Care Manager III ~ ., w ~-= ~-- ~_~~ a~o On May 19, 2005, Permanent Guardianship of Person for Mary C. Dugan was granted by Judge Kevin Hess to Cumberland County Office of Aging. Prior to this appointment Mrs. Dugan resided in her own home at 308 Mt. Allen Rd. Mechanicsburg, Pennsylvania. She was admitted to The Bridges at Bent Creek in Mechanicsburg on April 26, 2005 after the outcome of an Emergency Guardianship hearing. The Bridges at Bent Creek is an assistive living facility. At the time of the Permanent Guardianship hearing Brian Brooks of the PA Guardianship Association was appointed Permanent Guardian of Estate. Judge Hess on November 28, 2005 approved the withdrawal of PA Guardianship Association and appointed Keystone Guardianship Services as Permanent Guardian of the Estate of Mary Dugan. Mrs. Dugan continues to reside in the dementia unit at The Bridges known as The Garden. Her primary medical conditions of depression, hypertension, arthritis and GERD have remained stable. Mrs. Dugan continues to be seen by MD at Home at the facility on a monthly basis or as needed. This past year she was seen by her dentist Dr. Dennis Reid twice for routine cleaning. Mrs. Dugan continues to be transported by the facility due to her decreased cognitive impairment and high anxiety. Aging staff is present for all medical appointments. She also receives podiatry care on a routine basis as The Bridges have contracted with a podiatrist to make in facility visits on a routine basis. Mrs. Dugan has experienced several falls this year. Once she experienced a five minute episode of unresponsiveness and was taken to the Emergency Room. She was not admitted. The staff at The Bridges continues to provide hands on assistance with her activities of daily living, necessary verbal cues, redirection and 24 hour supervision. According to staff Mrs. Dugan requires more hands on assistance than she did a year ago. Mrs. Dugan also benefits from the activities that the dementia unit provides on a daily basis. Emotionally she has been fairly stable and has not displayed any significant behavioral problems. The goal is to keep her at Bridges at Bent Creek as long as possible since she likes it there. She attends all the different social activities on a daily basis. Mrs. Dugan enjoys her surroundings, the staff and the other residents. She is usually pleasant, has a smile on her face and enjoys joking around with others. She has made friends and feels very secure in her living environment. Ms. Dugan does not like being away for any length of time and is always relieved when she returns. She does not like being away from home. The staff enjoys having her and she is cooperative with most of their requests. Mrs. Dugan has no concerns or complaints regarding her living situation. Client is visited every 30-60 days by care management staff. The Bridges provides scheduling and transportation to all her medical appointments. The staff at The Bridges provides updates and notification of any changes that Mrs. Dugan might experience. Connie Stoneroad of Keystone Guardianship Services handles all of her financial affairs. The Office of Aging receives money from Keystone to purchase personal items for Mrs. Dugan. A few former neighbors continue to visit once in awhile and family who lives locally saw her once this year. Her daughter in Florida moved to Tennessee and continues to call twice a week on Sundays and Wednesdays. This daughter has noted a significant decline in her mother's cognitive abilities and conversation is very limited. Family is informed of any significant changes in Mrs. Dugan's life. It is the request of the Cumberland County Office of Aging that the Agency retains Guardianship of Person for Mary Dugan. The Bridges at Bent Creek has demonstrated an adequate quality of care for client by managing her daily needs and continuously monitoring her medical conditions. Mrs. Dugan's appearance is always good and she appears to be very content in her present living situation. Therefore, we believe that Mrs. Dugan should continue to reside at The Bridges of Bent Creek.
