Judge Edgar Bayley. About two weeks prior to the hearing, Mr. Weiser had been ta`~en
to the Carlisle Regional Medical Center by Middlesex Township Police. He had been
found in someone's property at 5:OOam wearing only boxer shorts and a trench coat.
At the hospital he was diagnosed with multi-infarct dementia, frost bite, ambulation
dysfunction and multiple abrasions and contusions on knees and feet.
After the Emergency Guardianship hearing, Mr. Weiser was taken to the Claremont
Nursing and Rehabilitation Center where he would be residing. On April 2, a final
guardianship hearing was held and the Agency was appointed the Permanent Plenary
Guardian of Person and Estate. After this, the Agency cleaned out Mr. Weiser's
apartment, paid his outstanding rent and applied for Medical Assistance.
Mr. Weiser adjusted very well to life at Claremont. He was pleasant and cooperative for
his care. He attended many of the activities. He especially likes the coffee hour, playing
trivia and Johnny Cash videos. He has had not negative behaviors. When the staff take
him to activities outside the facility, such as to a restaurant, he is very social and acts
appropriately.
During this past year, Mr. Weiser had cataracts removed from both eyes. He had no ill
effects of the surgery. Toward the end of 2009, Mr. Weiser began experiencing periods
of passing out which has been attributed to heart issues. Donald has begun to spend
more time in his room where he listens to music and relaxes. He seems to enjoy this
and there are no signs of depression. The nursing home staff also reports an increase
in his confusion. He was seen by both a podiatrist and a dentist during the year.
Mr. Weiser is usually visited every 30-60 days by care management staff. Staff also
attends all care plan meetings. The nursing home staff notifies the Agency's care
management staff of any changes or needs. Client has no living relatives.

It is the request of the Cumberland County Aging and Community Services that the
Agency retain the Guardianship of Person for Donald Weiser. The Claremont Nursing
and Rehabilitation Center has demonstrated an adequate quality of care for Donald by
managing his daily needs and continuously monitoring his medical conditions. Mr.
Weiser's appearance has always been good and he appears to be very content in hi
present living situation. We believe, therefore, that Mr. Weiser should continue to reside
at Claremont.
