Petition for the Appointment
of a Guardian for the Estate of a Minor
This petition is filed to request the appointment of a guardian for the estate of a minors
pursuant to section 5111 of the Probate, Estates and Fiduciaries code, 20 Pa.C.S. §5111:
1. The name of the minor for whose estate a guardian is requested is Garrett L. Steen.
The minor child is 5 years old and was born on March 11, 2004. The minor resides at
2880 Gettysburg Rd., Camp Hill, PA 17011.
The name of the second minor for whose estate a guardian is requested is Caitlyn G.
Steen. The minor child is 4 years old and was born on January 2, 2006. The minor
resides at 2880 Gettysburg Rd., Camp Hill, PA 17011..
2. The minor children do have a living parent.
The children's mother resides at 2880 Gettysburg Rd., Camp Hill, PA 17011.
3. This petition is filed by the following persons:

Mr. Reese is the maternal grandfather of the minor children. ~._

4. The petition has the consent of the following person or persons, and the consent of
each person is attached:
Elissa J. Steen, surviving parent of the minor children.
Kenneth Reese, maternal grandfather of the minor children.

5. The minors have no guardian of their respective estates; and the appointment of a
guardian for their estates is necessary for the following reason or reasons:
The minor is entitled to receive the death benefits from Assurant Employee Benefits,
Group Life Insurance policy number 5402904 on the life of their father with a face
amount of $75,000.00
6. The name and address of each proposed guardian, the age of any proposed individual
guardian, and the relationship of any proposed individual guardian to the minor, if
any, are as following, and the consent of each proposed guardian is attached:

Ms. Steen is the natural mother of the minor children.

Mr. Reese is the maternal grandfather of the minor children.
7. Except as stated below, any proposed individual guardian is 18 years of age or older,
has no interest that is adverse to that of the minors, and is not a fiduciary or surety, or
an employee of a corporate fiduciary or surety, of an estate in which the minors have
an interest, and any proposed corporate guardian is authorized to act as a fiduciary in
the Commonwealth, has no interest that is adverse to that of the minors, and is not a
fiduciary or surety of an estate in which the minors have an interest.
No Exceptions
8. The following is an itemization of the assets of the estate of the minors and their
location, approximate value, and income, if any:
Assurant Employee Benefits, Group Life Insurance policy number 5402904
$75,000.00.
9. Notice to the United States Department of Veteran Affairs is not required.
Wherefore, your petitioner requests that Elissa J. Steen be appointed as guardian
of the minor children's estate.
