The DILLSBURG AREA AUTHORITY hereby files its claim in the amount of Four
Thousand Seven Hundred Thirty-Six Dollars and Forty-Eight Cents ($4,736.48), plus costs of
Nineteen Dollars ($19.00), and reasonable attorney fees of Four Hundred Dollars ($400.00), for a
total of FIVE THOUSAND ONE HUNDRED FIFTY-FIVE DOLLARS AND FORTY-EIGHT
CENTS ($5,155.48), and sets forth in support thereof, the following:
1. The claimant is DILISBURG AREA AUTHORITY, of Dillsburg, York
County, Commonwealth of Pennsylvania, with its principal office located at 98 West
Church Street, Dillsburg, Pennsylvania 17019.
2. The name of the owner of the property against which this Municipal Lien is
filed is GORDON E. FRY JR., individually.
3. The property against which this Municipal Lien is filed is known and
municipally numbered as 212 Widders Drive, Mechanicsburg, MONROE
TOWNSHIP, Cumberland County, Pennsylvania 17055, being identified by
Assessment Parcel ID #22-29-2463-077.

4. The authority under which this Lien is filed is the Municipal Claims Act of
1923, May 16, P.L. 207, as amended (53 P.S. §7101, et seq.).

5. This Municipal Lien is filed to secure an interest for sewer rentals for Gordon
E. Fry Jr., as identified as Authority Account Number 4185, levied, assessed or
established for the period of time from December 17, 2009 through May 1, 2010,
together with interest and costs thereafter pursuant to the rules and regulations of the
Authority.
6. Statement of Claim: See attached Exhibit "A", reflecting details supporting a
claim for sewer rentals, which shows that $4,736.48 is owed, plus costs of $19.00 and
reasonable attorney fees of $400.00 for a total of $5,155.48.
7. The sewer rentals were duly established, levied, and assessed by the Dillsburg
Area Authority of Dillsburg, York County, Pennsylvania, being the lawfully
empowered body under the several Acts of Assembly of the Commonwealth of
Pennsylvania governing such matters, for the purpose of providing sewer service to the
property of the named Defendant.

Kindly issue a Writ of Scire Facias Sur Municipal Claim in the name of the Plaintiff, and
against the named Defendant for Municipal Lien 10-03429 MLD. The Writ should be issued
on the following terms:
A. Amount of claim: $2,496.02;
B. Interest and fees accrued from December 1, 2011;
C. Reason for filing claim: Defendants' failure to pay sewer rental charges;
D. Description of property: Single Family Residence at 212 Widders Drive,

WHEREAS, the Dillsburg Area Authority on the 24t' day of May 2010, filed its claim in our
Court of Common Pleas of Cumberland County, Pennsylvania at 10-03429 MLD, for the sum of
TWO THOUSAND FOUR HUNDRED NINETY-SIX DOLLARS AND TWO CENTS
($2,496.02) with interest from December 1, 2011, for sewer and water rentals, against the
following property situate in Monroe Township, Cumberland County, Pennsylvania, being
known and municipally numbered as 212 Widders Drive, Mechanicsburg, owned or reputed to
be owned by you;
AND WHEREAS, it is understood that the claim is still due and unpaid, and remains a

lien against the property;
NOW, YOU ARE HEREBY NOTIFIED to file your affidavit of defense to said claim, if
defense you have thereto, in the Office of the Prothonotary of our roue -L, within fifteen (15)

judgment may be entered against you for the whole claim, and the property described in the
claim will be sold to recover the amount thereof.
