TO THE HONORABLE, THE JUDGES OF SAID COURT:
The Petition of Idamae Ruth McClintock, by her attomey, Michael S. Travis, respectfully
represents:
1. Your petitioner was bom Idamae Ruth Reed, by marriage she took the name Idamae
Ruth McClintock.
2. Petitioner does not like her first name "Idamae," and has been accustomed to using the
name "Cookie" McClintock for as long as she can remember, and has used the name "Cookie
McClintock" professionally. Additionally, the name Idamae is difficult to pronounce and spell.
3. That petitioner has resided at 3 Timber Road, Mechanicsburg, Cumberland County,
Pennsylvania 17050 during the five years preceding the filing of this Petition.
4. That there are no judgements or decrees 0f like character of record against petitioner.
WHEREFORE, Petitioner requests this Court, pursuant to the Act of December 16, 1982,
P.L. 1309, No. 295 § 2, 54 Pa. Cons. Stat. § 702, for the following:
a. To fix a hearing on this Petition;
b. To direct that notice be given of this Petition and hearing;
c. To direct that the State Police conduct a fingerprint check pursuant to 54 Pa. C.S.A.
§702(b).
d. To enter a decree changing the petitioner's name from ldamae Ruth McClintoek to
Cookie Ruth McClintock.
