and in support of this Petition avers as follows: ' _

This petition is filed to request the appointment of a guardian for the estate of a minor pursuant to
section 5111 of the Probate, Estates and Fiduciaries Code, 20 Pa.C.S. §5111:
The name of the minor for whose estate a guardian is requested is Kathryn Marie Lester. The
minor is nine years of age and was born on September 17, 2000. The minor resides at 102 W.
Willow Street, Carlisle, Pennsylvania 17013.
The minor does have living parents. Constance M. Lester resides at 102 W. Willow Street,
Carlisle, Pennsylvania 17013. Frederick P. Lester resides at 2009 Powell Drive,
Chambersburg, Pennsylvania 17201.
This petition is filed by the following person: Constance M. Lester, 102 W. Willow Street,
Carlisle, Pennsylvania 17013, minor's natural mother.
4. The petition has the consent of the following persons, and the consent of each person is
attached: Constance M. Lester, Frederick P. Lester.
The minor has no guardian of her estate and the appointment of a guardian for her estate is
necessary for the following reason: The minor is entitled to receive the death benefits from
Banker's Life on the life of her grandfather with a face amount of $18,313.52.
6. The name and address of the proposed guardian, the age of the proposed individual guardian,
and the relationship of the proposed individual guardian to the minor, if any, are as follows,
and the consent of the proposed guardian is attached: Constance M. Lester, 102 W. Willow
Street, Carlisle, Pennsylvania, 17013, age 33, natural mother.
Except as stated below, any proposed individual guardian is 18 years of age or older, has no
interest that is adverse to that of the minor, and is not a fiduciary or surety, or an employee of
a corporate fiduciary or surety, of an estate in which the minor has an interest and any
proposed corporate guardian is authorized to act as a fiduciary in the Commonwealth, has no
interest that is adverse to that of the minor, and is not a fiduciary or surety of an estate in
which the minor has an interest.
There are no exceptions.
8. The following is an itemization of the assets of the estate of the minor and their location,
approximate value and income, if any:
Banker's Life account in the amount of $18,313.52
9. Notice to the United States Department of Veteran Affairs is not required.
10. Bond is requested to be waived in accordance with Pa.C.S. §5122 due to the circumstances of
the guardianship being requested in that the proposed guardian is minor's natural mother.
11. Due to the circumstances of the foregoing petition, that natural mother is requesting to be
appointed guardian of the minor's estate and there is no opposition to said petition it is
requested that this matter be decided on the merits with no formal hearing to be conducted by
the Court.
WHEREFORE, your petitioner requests that Constance M. Lester be appointed as guardian
of the minor's estate.
