PETITION FOR CHANGE OF NAME
AND NOW, comes Petitioner, Canio Anthony Salese, by and through his attorney, Dirk
E. Berry, Esquire, and respectfully requests that this Honorable Court order a change of name for
Petitioner pursuant to 54 Pa.C.S.A. §701 et seq. and in support thereof respectfully represents as
follows:
1. Petitioner is a resident of Cumberland County, Pennsylvania, and he has resided at the
l~ollowing location during the five years preceding the filing of this petition:
403 Arch Street, Carlisle, Cumberland County, Pennsylvania 17013
2. Petitioner respectfully requests this Honorable Court change his name to CANIO
JOHN SALESE.
3. There are no judgments or decrees of record against Petitioner.
WHEREFORE, Petitioner respectfully requests this Honorable Court order a change of
name for Petitioner pursuant to 54 Pa.C.S.A. §701 et seq.

PETITION FOR NAME CHANGE
AND NOW, this ~ day of February, 2001, comes Petitioner, Janet Marian College,
by and through counsel, Lawrence J. Rosen, Esquire and offers the following averments in
support of the within Petition:
Petitioner, Janet Marian College (SSN: 262-84~1264), is an adult female residing
at 10vis Drive, Mechanicsburg, Pennsylvania, 17055.
Petitioner was bom Janet Marian Plummer on December 3, 1944, in the Canal
Zone in the Country of Panama while her father was in the United States military.
(See Exhibit A).
All of her life, Petitioner has used the name Jeannette believing same to be her
given name. (See Exhibits B and C).
On or about June, 2000, Petitioner was preparing to go on a cruise with her
husband and required a copy of her birth certificate which she was able to obtain
through the State Department.
Upon receipt of said Birth Certificate, Petitioner learned for the first time that her
given name was Janet and not Jeannette.
The discrepancy between Petitioners's Birth Certificate and her other
identification such as her driver's license has created and will continue to create
problems when she wishes to travel abroad or in such situations in which a birth
certificate may be required as positive proof of identification.
Petitioner is seeking an Order from Your Honorable Court to the eft~ct that
henceforth Janet Marian College shall be known as and may use the nmne
Jeannette Marian College.
WHEREFORE, Petitioner asks that Your Honorable Court grant the relief requested
herein and issue an Order authorizing the change of Petitioner's first name from Janet to
Jeannette.
