PETITION FOR CHANGE OF NAME PURSUANT
TO 54 PA. C.S.A. § 701 ET SEC .
The Petitioners, MICHAEL E. ENCK and STEPHANIE E. ENCK, through their undersigned
counsel, request that this Honorable Court order a Change of Name for their minor adopted child pursuant to
54 PA. C.S.A. § 701 et seq., and in support thereof represent as follows:
1. Petitioners, MICHAEL E. ENCK and STEPHANIE E. ENCK, husband and wife, are adult
individuals residing at 1829 Willow Road, Camp Hill, Cumberland County, Pennsylvania, 17011.
2. Petitioners are the adoptive parents of NING LIWEN (DOB August 31, 1999) by a
Certification and Registration of Adoption dated June 27, 2000. The Certificate and Registration of Adoption
was entered pursuant to the laws of the Jiangi Province and the Peoples Republic of China. A true and
correct copy of said Certificate and Registration of Adoption is attached hereto and incorporated herein as
Exhibit "A". A true and correct copy of the Birth Certificate issued by the Jiangi Province, Peoples Republic
of China is attached hereto and incorporated herein as Exhibit "B".
3. Since July, 2000, the minor child, NING LIWEN, has resided with the Petitioners at 1829
Willow Road, Camp Hill, Cumberland County, Pennsylvania 17011.
4. The Petitioners were unable to change the name of the minor child during the adoption
proceeding in China and wish to do so pursuant to the law of the Commonwealth of Pennsylvania.
5. It would be in the minor child's best interest that the minor child's name be changed from
NING LIWEN to LIBERTY ELAINE ENCK to strengthen the bonds between the minor child and the adoptive
parents.
6. At the age of eighteen (18) months, the minor child is unable to understand the significance of
such a name change.
7. Due to the age of the minor child, the fact that the identity of her parents in China is unknown
and the adoption proceeding through which the name was not changed under Chinese law, the Petitioners
would ask that your Honorable Court consider a waiver of the publication requirements contained in
C.C.R.P. 5.
WHEREFORE, Petitioners request that this Court enter a decree changing the name of the minor
child, NING LIWEN to LIBERTY ELAINE ENCK. Further, Petitioners request that this Honorable Court
schedule a hearing as provided by local rule and that Petitioners be relieved of the responsibility of making
publication pursuant to C.C.R.P. 5.
