Petitioner, Glen Ross Goudy, Jr. was born on August 27, 1975, in Harrisburg, Dauphin County, Pennsylvania. Petitioner's name at birth was Glen Ross Goudy, Jr. An original birth certificate is attached hereto as Petitioner's Exhibit "A" and incorporated herein by reference. 3. Petitioner currently resides at 427 Falrview Avenue, Enola, Cumberland County, Pennsylvania, 17025. 4. Mr. Goudy received the following social security number under the name Glen Ross Goudy, Jr.: 193-54-7805. He has used this social security number for employment and tax purposes throughout his life. 5. Petitioner seeks to change his name to Lee Loisell, for the following reasons: a) Petitioner's birth name was Glen Ross Goudy, Jr.; Sr.; b) Petitioner was named after his biological father, Glen Ross Goudy, c) Glen Ross Goudy, Sr. was physically and emotionally abusive to Petitioner; d) Petitioner has not spoken with Glen Ross Goudy, Sr. for at least seven years, and does not acknowledge him as his father. e) Petitioner considers his step-father, Douglass Loisell, to be his father, and Mr. Loisell has treated Petitioner as his son for the past eight (8) years. f) Petitioner has been using the name Lee Loisell, for social purposes for the last year and a half; g) Petitioner has never used the name Lee Loisell on any legal documents, but would like to use the name exclusively for all social, business, and legal purposes. 6. Petitioner's fingerprints are attached hereto as Exhibit "B" as required by 54 Pa. C.S. § 702(b).
