Petitioner is now desirous of changing her name to Kristy Martson. 4. Prior to residing at 303 West 1 St Street, Boiling Springs, Cumberland County, Pennsylvania 17007. Petitioner has resided in the following locations over the last five (5) years; (a) 500 South Spring Garden Street, Carlisle, Pennsylvania from 2002-2007 5. Petitioner is requesting a change of name as indicated above because she wishes to drop former husband's surname. 6.
