The Petitioner, Drew Kennerly-Becker, requests that this Court or---a
change of name for him and in support thereof represents the following:
1. Petition is an adult individual, whose date of birth is February 25,
1992 and who presently resides at 35 Honeysuckle Dr. in Cumberland County
Mechanicsburg, Pennsylvania.
2. Petitioner desires this change of name for the following reasons:
a) Kennerly is the last name of Drew's biological father;
b) On average, Drew has only seen his biological father 6
hours per month for the past 18 years.
c) Drew's biological father has active in raising him;
d) Drew has never resided in the same household as his
biological father;
e) Although the Kennerly family is local, Drew rarely has any
contact with them;
f) Drew has become the young man he is today due to the
unconditional love and attention he has received from his
mother, his grandparents and his uncle. These individuals

3. Petitioner is a citizen of the United States of America.
4. There are no judgments or decrees pending against the petitioner.
WHEREFORE, Petitioner respectfully requests that this Court enter an
Order changing the name of Petitioner from Drew Kennerly-Becker to Drew
Becker.

for Change of Name is scheduled for the ~~ day of

Cumberland County Courthouse, One Courthouse Square, Carlisle,
Pennsylvania.
Petitioner is directed to give notice of the hearing by publication in the
Cumberland Law Journal and one newspaper of general circulation in
Cumberland County, once a week for two consecutive weeks.
Petitioner will obtain certifications of actions filed or record from Clerk of
Courts and instruments of indebtedness from Prothonotary and Recorder of
Deeds dated no more than two days prior to the hearing.
