Petitioner Brent Eugene Ash is a resident of Enola, Cumberland County, Pennsylvania and has resided at thc following address for more than the last five years: 5404 Lcgen Lane, Enola, Pennsylvania 17025. 2. The Petitioner was born on August 15, 1978 in Harrisburg, Dauphin County, Pennsylvania. His parents are LeRoy Eugene Ash (father) and Wanda Ann Ash (nee: Rothrock) (mother). 3. Petitioner's mother is a member of his household, and Petitioner's father lives at 746 Erford Road, Camp I-lill~ Cumberland County, Pennsylvania. 4. Petitioner would like to change his name from Brent Eugene Ash to Brent Tala Rothrock for the following reasons: a.) Petitioner would like to change his sumome from Ash to Rothrock bemuse Rothrock is his mother's maiden name, and Petitioner wishes by that change to reflect the fact that his mother has raised and nurtured and assisted him throughom his life, whereas his father has exercised minimal involvement in Petitioner's life, despite the fact that numerous entreaties have been made to the father to become more involved in the Petitioner's life; b.) The Petitioner would like to change his middle name from Eugene to Tala because he does not want his middle name to be the sameas his father's middle name, and has chosen Tala as his new middle name because that name is appealing to the Petitioner. 5. Your Petitioner is not married and has not fathered any children in his lifetime. 6. There are no judgments or decrees of like character of record against the Petitioner. WHEREFORE, Petitioner requests this Honorable Court to set a hearing to consider the requested change ofnan~ as contained herein, to ultimately enter an Order changing Petitioner's name from Brent Eugene Ash to Brent Tala Rothrock, and to provide any other relief this Court deems appropriate.
