PETITION FOR CHANGE OF NAME
TO THE HONORABLE JUDGES OF SAID COURT:
AND NOW COMES Petitioner, Joshua Jacob Steiner-Cothard, by and through his attorney,
Shaun E. O'Toole, and represents as follow:
1. Petitioner is Joshua Jacob Steiner-Cothard, who resides at 60 Hummel Avenue, 1St Floor,
Lemoyne, Cumberland County, Pennsylvania 17043.
2. Petitioner's date of birth is January 13, 1992.
3. Petitioner has resided at the following addresses for the five (5) years previous to the
filing of this Petition:
(a) 60 Hummel Avenue, 1St Floor, Lemoyne, Cumberland County, Pennsylvania 17043,
for the past (3) years; and
(b) 40 South Baltimore Street, Apt. C, Dillsburg, York County, Pennsylvania 17019, for
the two (2) years prior to that.
4. Petitioner desires and intends to change his name to Joshua Jacob Cothard.
5. Petitioner is eighteen (18) years old.
6. Petitioner provides the following reasons for the request:
(a) To honor his mother, Aime Cothard, who assumed the primary responsibility of
raising Petitioner; and

(b) Petitioner is one of only a few remaining male Cothards and the name change will
enable him to carry on the Cothard name.
7. The proposed change in Petitioner's name, if granted, will not be detrimental to the
interests of any other person and is not against public interest.
8. The Petition is not filed to defraud creditors.
9. WHEREFORE, Petitioner requests the Court, pursuant to the Act of December 16, 1982,
P.L. 1309, No. 295 §2, 54 Pa. C.S. §701, for the following:
a. To fix a hearing on this Petition;
b. To direct that notice be given on this Petition and hearing; and
c. To enter a decree changing the Petitioner's name from Joshua Jacob Steiner-Cothard
to Joshua Jacob Cothard.

against the Petitioner shall be submitted at the hearing.
It is further ordered that a notice of said hearing shall be made by one (1) publication in
both the Cumberland Law Journal and in a newspaper of general circulation in Cumberland
County, Pennsylvania. Proof of publication shall be submitted prior to or at the hearing.

Pa.C.S.A. § 701 have been satisfied, it is hereby ORDERED and DECREED that the name of
Petitioner, Joshua Jacob Steiner-Cothard, is hereby changed to Joshua Jacob Cothard and that
Petitioner, from this date forward, shall exclusively use and be known by the name of Joshua
Jacob Cothard.
