Keller-Revist LLC, files this lien against Elena B. Duzs, owner and against the building
hereinafter described and the curtilage appurtenant thereto for the payment due to him as claimant in the
erection and construction of said building, and makes the following statement of his demand:
1. The name of the claimant is Keller-Revist LLC, with an address of
6935 Brookville Rd. Indianapolis IN 46239
2. The name of the owner or registered owner of the property subject to the lien is Elena B.
Duzs 147 S. College St., Carlisle PA 17013
3. Claimant contracted with eComfort USA 876 Eagle Drive Bensenville IL 60106,
Contractor.
4. The materials and/or labor referred to were furnished by virtue of an oral agreement and
consisted of boiler tanks and parts, related materials and/or labor as indicated by the
invoice(s) attached and marked as Exhibit A and made a part hereof.
5. The amount claimed to be still due and chargeable against the premises hereinafter
referred to is $1,600.00 as of May 7, 2010. The claimant has note or other collateral
security for his claim.
6. The first materials/labor were furnished by claimant on January 14, 2010 and the last
materials/labor were furnished on May 7, 2010.
7. Written notice of the intention to file this claim in accordance with the requirements of the
Act of Assembly in such case, was duly served upon said owner on June 4, 2010 _at 147
S. College St. Carlisle PA 17013.
8. The aforesaid materials/labor were furnished in and about the original erection and
construction of the property, commonly known as 147 S. College St., Carlisle PA 17013
as more fully described as follows:
Parcel ID: 04-21-0320-304, more fully described in a deed recorded as 200735269,
all in the Borough of Carlisle, County of Cumberland, State of Pennsylvania
This lien is claimed from January 14, 2010 when the first visible work was commenced
and against the fee simple interest of the owner in the aforesaid premises.
