SUGGESTION OF NONPAYMENT AND AVERMENT OF DEFAULT
AND NOW, July 6, 2010, the Commonwealth of Pennsylvania, Department of
Labor and Industry to the use of the Unemployment Compensation Fund,
Plaintiff-Claimant herein, suggests of record that the above claim is still
due and owing to the Claimant, and avers that the above-named Defendant is
still in default for nonpayment thereof. The Prothonotary is directed to
enter this suggestion and averment on the proper docket of said claim, and
also to index it in the judgment index for the purpose of continuing the lien
of said claim.

TO THE PROTHONOTARY OF SAID COURT:

NOW, to wit, March 15, 2012, the Commonwealth of Pennsylvania,
Department of Labor and Industry, to the use of the Unemployment Compensation
Fund, Plaintiff in above action, through its Chief Counsel, by its Deputy
Chief Counsel for Unemployment Compensation, acknowledges having received of
the Defendant above named full payment and satisfaction of the above
captioned Lien, with the interest and costs thereon due it; and desire that
satisfaction be entered upon the records thereof.
AND you, the Prothonotary of said Court, upon receipt by you of your
costs, are hereby authorized and empowered, in the name and stead of the
Plaintiff, to enter full satisfaction upon the record of said Lien as fully
and effectually, to all intents and purposes, as we could were we present in
person to do so; and for so doing, this shall be your sufficient warrant of
authority.
