Defendant LISA GOODRICH is an individual residing at: 1595 W LISBURN RD, MECHANICSBURG, PA 17055. 4. Upon application by Defendant(s) a Credit/Revolving Charge Account was established for Defendant(s), which was assigned account number 5291497344903930. 5. Defendant(s) subsequently utilized the aforesaid account to make various purchases and/or receieve cash advances, thereby incurring payment obligations to Plaintiff and/or Plaintiff's predecessor in interest. 6. Defendant(s) defaulted by failing to adhere to repayment terms and in accordance therewith, the entire remaining balance became due and immediately payable. 7.
