DECREE
AND NOW, this ~ day of 2010, upon consideration of the
Petition for Approval of Minor Settlements, it is hereby ORDERED and DECREED that the
Petitioner is authorized to enter into the third party case settlement for Konstance Gregory in the
amount of $3,000.00 and Leah Gregory in the amount of $22,500.00. Petitioner is authorized to
sign the Releases for these settlements.
The settlement amount shall be distributed as follows:
(1) TO: Anita Martin, as Parent and Natural Guardian of Konstance Gregory, a
Minor, $2,018.71 to be deposited into a restricted, federally insured account or certificate of
deposit marked "not to be withdrawn, assigned, negotiated or otherwise alienated until
Konstance Gregory reaches her majority on September 5, 2012, except upon prior Order of the
Court";
(2) TO: Metzger, Wickersham, P.C., $750.00 for counsel fees on Konstance
Gregory's claim;
(3) TO: Metzger, Wickersham, PC., $231.29 for reimbursement of costs on
Konstance Gregory's claim;

(4) TO: Anita Martin, as Parent and Natural Guardian of Leah Gregory, a Minor,
$16,564.00 to be deposited into a restricted, federally insured account or certificate of deposit
marked "not to be withdrawn, assigned, negotiated or otherwise alienated until Leah Gregory
reaches her majority on October 11, 2010, except upon prior Order of the Court";
(5) TO: Metzger, Wickersham, P.C., $5,625.00 for counsel fees on Leah Gregory's
claim;
(6) TO: Metzger, Wickersham, PC., $311.00 for reimbursement of costs on Leah
Gregory's claim;
Counsel shall file, within thirty (30) days from the date of the distribution of the
settlement funds, proof of such deposits.

AFFIDAVIT OF DEPOSIT OF MINORS' FUNDS
The undersigned, counsel for Plaintiffs, hereby certifies that the net settlement
amount of $2,018.71 as set forth in this Court's Decree dated July 28, 2010 was deposited
into a restricted, federally insured account on August 30, 2010, at Members 1St Federal
Credit Union in the name of the minor Konstance Gregory marked "No withdrawals prior
to September 15, 2012 (age 18) without prior court approval." Proof of deposit is
attached hereto as Exhibit "A".
The undersigned, counsel for Plaintiffs, hereby certifies that the net settlement
amount of $16,564.00 as set forth in this Court's Decree dated July 28, 2010 was
deposited into a restricted, federally insured account on August 30, 2010, at Members 1St
Federal Credit Union in the name of the minor Leah Gregory marked "No withdrawals
prior to October 11, 2010 (age 18) without prior court approval." Proof of deposit is
attached hereto as Exhibit "B".
