On May 1, 2009, Temporary Gaurdianship was granted to the Office of Agt~g. Following
the hearing Ms. Chronister was admitted to the Claremont Nursing and Rehabilitation
Center. Clothes were purchased for her by the Office of Aging, including sneakers,
slippers, socks, 2 pair pajamas, robe, 4 outfits and bras. She was very pleased with the
new clothes. She adjusted quickly to her new surroundings.
On May 26, 2009 The Office of Aging was granted Permanent Guardian of Person.
During the year Ms. Chronister was diagnoses with pneumonia. She was treated with
intermuscular injects of Lasix. When visited by staff a few weeks later she was feeling
fine. She has had a good appetite and her weight increased from 90 pounds upon
admission to 138 pounds currently. The Home has had to adjust her meals because she
snacks so much.
At one point during the year, Ms. Chronister became obsessed with the idea that her
daughter, Kim, who is in prison, was killed in an automobile accident. We verified with a
friend of the daughter that she was still in prison and let the Home staff know this so
they could give this information to Winifred. She received a letter from a sister that lived
in Lansing Michigan that we did not know about. I called the sister and spoke to her,
advising her of the guardianship and the name and address of the nursing home. She
will notify another sister who lives in Florida.
Ms. Chronister has had ambulation problems but refused to use a walker. Physical
therapy had been ordered but she refused to participate. She likes to socialize and
attends most activities although does not directly participate in them. On May 11, 2010
Ms. Chronister fell in her room and suffered a fractured hip. We gave permission to the
surgeon to do the surgery as well as for a blood transfusion if necessary. Dr. Healy
reported after the surgery that Winifred came through fine. She was returned to
Claremont 2 days later. During the day, she is usually in a recliner. She is receiving
physical therapy to see if she can again ambulate. Also during the year she has had
several urinary tract infections which effects her behavior.

Ms. Chronister was visited by staff every 30 - 60 days. Staff also attended care plan
meetings at the Home.
It is the request of the Cumberland County Aging and Community Services that the
agency retain Guardianship of Person for Ms. Chronister. The Claremont Nursing and
Rehabilitation Center has demonstrated an adequate quality of care for her medical
problems. We believe, therefore, that Ms. Chronister should continue to reside in that
nursing facility.
