Petitioner, Stephanie Hollen, respectfully states:
Petitioner is Stephanie Hollen, an adult individual who resides at 3800 Goifview Dr,
Mechanicsburg, PA 17050.
The minor child, BELLA BREEDER, was bom on August 13, 2009, and is currently

eleven (11) months old.
Petitioner is the minor child's maternal grandmother.
The minor child lives at 3800 Golfview Dr, Mechanicsburg, PA 17050, with the proposed
guardian, Stephanie Hollen.
The mother of the minor child is Chelsea Sheeder, who resides at 127 N. Main St. Apt. 2
Chambersburg, PA 17201
The father of the minor child is Jon Bevins, who resides at 200 N Middlesex Rd, Carlisle,
PA 17013.
The child is in need of a guardian because the child's mother abandoned or stopped
taking care of her; the child's mother failed to seek necessary medical treatment for her;
the child's mother failed to provide suitable housing for her; She is! has been living
house to house with no stability for child, has no income, no job, did not finish high
school or GED ,has not followed up for doctor referrals or child's current shots. Did not
interact with child . Has no drivers license or vehicle; the child's mother is a drug and
alcohol abuser. she as has many psychological exams in the past and is prescribed
medicine which she does not take and can become very violent. the child's father does
not have the child for the the following reason: He has three other children, and has no
interest in having Bella full-time. Jon does not have his own housing and not stable; a
physician or other medical provider is requiring that the proposed guardian have legal
authority to obtain medical treatment for the child; the proposed guardian wishes to have
general legal authority to be able to protect and provide for the child in any
circumstances which may arise in the future; and the following reason: So I have the

authority to make decisions based upon child's best interest. Such as but not limited to,
health care, child care, education.

Person of the minor child.

WHEREFORE, Petitioner, Stephanie Hollen, nominates herself as Guardian of the
Person of BELLA SHEEDER, and requests this Honorable Court to make such appointment
pursuant to Section 5111 of the Probate, Estates and Fiduciaries Code, 20 Pa.C.S.A. § 5111.
