DECREE
And now, this ~~ day of , 2010, upon consideration of the
attached Petition, it is hereby ORDERED and DECREED as follows:
(a) TD Bank, N.A., (the "Trustee") is hereby authorized to allocate all
annuities received on behalf of the Kenneth Stites Special Needs Trust
(the "Trust") to the income of such Trust;
(b) The Trustee is hereby authorized to expend up to $100,000 of the principal
or income of the Kenneth Stites Special Needs Trust to modify and
improve the Minor's home; and
(c) The Trustee is authorized to expend principal of the Trust in the amount of
$2,500 to pay The Law Office of Jane K. Anastasia, LLC for its legal
services related to this petition.
