Wolf, Esquire, hereby presents this petition requesting that this Court order a change of petitioner's name, pursuant to 54 Pa.C.S. § 701 et seq., and in support thereof makes the following averments: 1. Petitioner was born on April 21, 1981, and is over the age of 18 years. 2. During the five years preceding the filing of this petition down to and including the date thereof, petitioner has resided continuously at the following locations: (a) 829 Nesbit Drive, Carlisle, Pennsylvania 17013 presently and either 560 West North Street, Carlisle, Pennsylvania 17013 or 829 Nesbit Drive, Carlisle, Pennsylvania 17013 from approximately August 2006 -Present; and (b) 560 West North Street, Carlisle, Pennsylvania 17013 - 2005-2006. 3. There are no judgments or decrees of record or any other matters of like character against the petitioner. Certification of an official search is attached hereto as Exhibit "A„ 4. Pursuant to 54 Pa.C.S. §702, Petitioner has attached a set of his fingerprints hereto as Exhibit "B". 5. The reasons for the requested change of name are the following: (a) Petitioner came to the United States in 2000 and since being in the United States people have mispronounced his name and made fun of his name; (b) In June, 2010, Petitioner became a United States citizen and wishes to change the spelling of his first name due to the mispronunciation; and (c) Petitioner's daughter's middle name is Anees, given in Petitioner's honor and Petitioner wishes to change the spelling of his first name to match the spelling of his daughter's middle name. 7. If approved, Petitioner's request will merely change the spelling of his first name. Petitioner requests that the hearing on his petition be scheduled prior to October 20, 2010 as he will be out of the country for a trip beginning October 20, 2010. WHEREFORE, petitioner, Anis Abu-Aiadeh, respectfully prays that this Honorable Court enter an order fixing a time for a hearing on the instant petition before this Court, no less than one month, nor more than three months from the filing of the instant petition, but prior to October 20, 2010, and that after hearing, that this Honorable Court issue an Order authorizing the change of petitioner's name to Anees Abu-Aiadeh, along with any additional relief that the Court may deem appropriate. Mr. Abu-Aiadeh owns property in Carlisle, but there is no mortgage recorded under his name. Mr. Abu-Aiadeh does not owe delinquent taxes.
