PETITION OF TRAVIS HUNTER WILLS FOR THE CHANGE OF HIS NAME
TO ALEXIS LYNN DOVE
1. Petitioner Travis Hunter Wills is a resident of Camp Hill, Cumberland County,
Pennsylvania and has resided at the following address since approximately, July, 2009:
1191 Shoreham Road, Camp Hill, Pennsylvania 17011.
2. The Petitioner was born on March 24, 1987 in Camp Hill, Cumberland County,
Pennsylvania. A certified copy of Petitioner's birth certificate is attached hereto, marked
Exhibit "A" and incorporated herein by reference.
3. Petitioner would like to change his name from Travis Hunter Wills to Alexis
Lynn Dove, in that Petitioner is undergoing gender reassignment, and Petitioner has
chosen the name Alexis Lynn Dove for the following reasons:
a. with respect to the Alexis, Petitioner's mother suggested the name and Petitioner
thinks it is wonderfully appropriate; :iga.oo Po ATty

b. with respect to the name Lynn, as disclosed on the attached birth certificate,
Lynn is the Petitioner's mother's middle name and
c. with respect to the name Dove, as also shown by the attached birth certificate,
it is Petitioner's mother's maiden name, and she would like to honor that name by taking
the name.
4. Petitioner is not married and has not had any children in his lifetime.
5. Petitioner has no criminal record, a fact we anticipate being substantiated by the
standard criminal record check that will be run as a result of the filing of this Petition.
6. There are no judgments or decrees of like character of record against the Petitioner.
7. Petitioner's mother fully supports the request contained is this Petition; it is possible
that Petitioner's father does not support this request, although Petitioner has had little
contact with his father for approximately the last 10 years; we have Petitioner's father's
address and, should the Court deem it necessary or advisable, we will provide written
notice of the hearing date on this Petition to Petitioner's father.
WHEREFORE, Petitioner requests this Honorable Court to set a hearing to consider
the requested change of name as contained herein, to ultimately enter an Order changing
Petitioner's name from Travis Hunter Wills to Alexis Lynn Dove, and to provide any other
relief that this Court deems appropriate.
