AND NOW, this ~! of 2010, to judicially resolve Petitid~er's request
for the appointment of guardian, the Court does hereby ORDER AND DECREE th ollowing:
1. The Court hearing on the attached petition is scheduled for ~ ~ ,

Carlisle, Pennsylvania.

2. The Court directs the issuance of an appropriate citation with rule td show cause
why the above-captioned individual should not be adjudged an incapacitated persoh and why the
Court should not appoint Petitioner as guardian of his person and of his estate.
3. Petitioner shall cause to be served by personal service the citations l and petition
with the attached important notice upon the alleged incapacitated person at least twle~nty (20) days
prior to the Court hearing. The contents and terms of the petition and notice sha11 ~be explained
to the maximum extent possible in language and terms the alleged incapacitated ~rson is most
likely to understand. An affidavit of service shall be filed before the hearing o~ bff~red as an
exhibit at the beginning of the hearing.
4. Petitioner shall notify the Court, in writing, at least seven (7) dajy~ prior to the
hearing if counsel has NOT been retained by or on behalf of the alleged incapajc~tal~ person.
This notice shall also contain all pertinent information which indicates whetherdr not oounsel
should be appointed to represent the alleged incapacitated person.

5. The alleged incapacitated person shall be present at the Court hearing unless it is
established that his physical or mental condition would be harmed by his presence, or it is
impossible for him to be present because of his absence from the Commonwealth.
