AND NOW, Patty J. Daron, being duly sworn according to law, does depose and state:
1. That she is a competent adult and mother of Dustin S. Hockensmith,~ the alleged
incapacitated person in the above-captioned action. !,
2. That she served a copy of the Petition for Appointment of Emergency loan of the
Estate and Person in Accordance with 20 Pa.C.S.A. § 5513, and read the Im rtant Notice
Citation to Dustin S. Hockensmith, on the ~ day of ~L~P1'~111YS~'12 2010, by d-delivery
to him at 3166 Ritner Highway, Newville, Cumberland County, Pennsylvania.
I verify that the statements made in this affidavit are true and correct. I and d that
